Evidenz aus Krebsregisterdaten zur Behandlung des Duktalen Karzinoma in Situ (DCIS) der weiblichen Brust

Mittels Krebsregisterdaten aus mehreren Bundesländern Deutschlands analysieren wir reale Versorgungspfade nach einer DCIS-Diagnose und deren Einfluss auf das Risiko für nachfolgende invasive Karzinome in der Brust. Ziel ist es, das Potenzial zur Therapie-Deeskalation im klinischen Alltag evidenzbasiert zu bewerten sowie patientenrelevante Einflussfaktoren auf den Krankheitsverlauf zu identifizieren. Die Analyse der Daten des Krebsregisters Brandenburg-Berlin ergab erste interessante Resultate. Eine Erweiterung auf weitere Krebsregister ist geplant.

Register zu Komplikationen beim interventionellen Ultraschall (INVUS)

Das Register für Interventionellen Ultraschall (INVUS) ist eine deutschlandweite prospektive Beobachtungsstudie, die vom Arbeitskreis für Interventionellen Ultraschall (DEGUM) in Kooperation mit dem Brandenburgischen Institut für Klinischen Ultraschall (BIKUS) entwickelt worden ist. Ziel ist der Aufbau eines systematischen Datenpools zu ultraschallgestützten diagnostischen und therapeutischen Eingriffen verschiedener Fachdisziplinen.

Wir unterstützen die Auswertung der erhobenen Daten u. a. mit der Analyse von Komplikationsraten, Risikofaktoren für Komplikationen sowie Einflussfaktoren für den Erfolg der Intervention – mit besonderem Fokus auf Spätkomplikationen wie Infektionen. Darüber werden Versorgungsrealitäten abgebildet und eine wissenschaftliche Grundlage für zukünftige Leitlinien geschaffen.

 

Behandlung und Spätfolgen des Hodgkin-Lymphoms bei Kindern

Wir unterstützen die Vereinigung für Pädiatrische Onkologie und Hämatologie (GPOH) beim Aufbau eines Registers zu den Spätfolgen der Behandlung des Hodgkin-Lymphoms bei Kindern und Jugendlichen. Ziel ist die Erfassung von Spätfolgen bei in klinischen Studien behandelten Patienten zur Bewertung der späten Nebenwirkungen verschiedener Behandlungen. Dadurch können Behandlungen hinsichtlich ihrer Heilung und des Potenzials für Spätfolgen bewertet werden, um die optimale Behandlung auszuwählen.

Darüberhinaus arbeiten wir mit der EuroNet Paediatric Hodgkin’s Lymphoma Group (EuroNet-PHL), an der sich 20 Länder beteiligen, am Design klinischer Studien, die neue Therapieformen evaluieren. Diese bilden die Basis für evidenzbasierte künftige Behandlungsstrategien mit einem günstigen Profil von Heilung und Spätfolgen. Das Projekt wird aus Mitteln der GPOH finanziert.

 

Evaluation einer palliativmedizinischen Tagesklinik (SEELE)

Um die Krankenhausaufnahme von Palliativpatienten zu reduzieren, werden in der SEELE Studie an mehreren Standorten palliativmedizinische Tageskliniken eingerichtet und Palliativpatienten dort versorgt. Neben einer Reihe medizinischer Verrichtungen, die sonst eine Krankenhauseinweisung erfordern würden, wird ein breites Spektrum unterstützender Verfahren angewandt von einem multiprofessionellen Team (Palliativmedizin, Palliativpflege, Physiotherapie, Ergotherapie, Logopädie, Klangtherapie, Aromapflege, Psychoonkologie, Sozialdienst, Ernährungsberatung, Seelsorge, Schmerztherapie). Wir evaluieren diese neue Versorgungsform durch einen Vergleich der Krankenhausaufenthalte und anderer Endpunkte der eingeschlossenen Patienten mit Kassendaten von Patienten ohne palliativmedizinische Tagesklinik. Das Projekt wird finanziert vom Innovationsfonds des Gemeinsamen Bundesausschusses (GBA).

 

Target Trial Emulation von Durvalumab in der Lungenkrebstherapie (REGINT)

Im Rahmen des Projekts REGINT, das eine Bestandsaufnahme von registerbasierten Interventionsstudien in Deutschland erstellt und Handlungsempfehlungen für deren Durchführung entwickelt, führen wir zwei Target Trial Emulationen durch und benutzen dazu klinische Daten aller deutschen Krebsregister. Wir evaluieren den Effekt einer Therapie mit Durvalumab bei kleinzelligem und nicht-kleinzelligem Lungenkarzinom. Das Projekt wird vom Bundesgesundheitsministerium (BMG) gefördert.

 

Gesundheitliche Versorgung von Menschen mit Demenz in Deutschland (REGIODEM)

Im Projekt REGIODEM analysieren wir gemeinsam mit mehreren Partnern umfangreiche Daten dreier großer gesetzlicher Krankenkassen zu Menschen mit Demenz. Ziel des Projektes ist die Darstellung der gesundheitlichen Versorgung von Menschen mit Demenz in Deutschland. Von besonderem Interesse sind hierbei regionale Unterschiede bezüglich der Inanspruchnahme und Kosten von Gesundheitsleistungen. Das Projekt wird finanziert vom Innovationsfonds des Gemeinsamen Bundesausschusses (GBA).

Evaluation eines neuartigen Rehabilitationskonzeptes zur stationären Versorgung pflegebedürftiger Menschen in Deutschland (SGB Reha)

In der multizentrischen Studie SGB Reha im randomisierten kontrollierten Stepped-Wedge-Design evaluieren wir ein neuartiges Rehabilitationskonzept zur stationären Versorgung pflegebedürftiger Menschen in Deutschland. Basierend auf einem Best-Practice-Modell sind die Kernelemente des Konzepts die sektorenübergreifende und interdisziplinäre Versorgung durch die Professionen Medizin, Pharmazie und Sozialpädagogik. Ziel ist es, Alltagsfertigkeiten, kognitive Leistungen sowie die Lebensqualität der Betroffenen zu verbessern sowie die Anzahl eingenommener Arzneimittel und erforderlicher Krankenhausaufenthalte sowie dadurch entstehende Kosten zu verringern. Management der Axilla nach neoadjuvanter Chemotherapie zur Behandlung von Brustkrebs (AXSANA)

AXSANA (Axillary Surgery After Neoadjuvant Treatment) ist eine prospektive multizentrische Registerstudie zur Bewertung verschiedener leitlinienkonformer Operationsverfahren in der Axilla nach neoadjuvanter Chemotherapie für Brustkrebs. Endpunkte der Studie sind onkologischer Outcome sowie Lebensqualität und Armmorbidität der Teilnehmer. Das Projekt wird finanziert von der European Breast Cancer Research Association of Surgical Trialists (EUBREAST) und der Claudia von Schilling Foundation for Breast Cancer Research.

Register der Sinne (Hören, Riechen, Schmecken)

In Zusammenarbeit mit dem Zentrum für Klinische Studien (ZKS) planen wir den Aufbau eines Registers der Sinne in der älteren Bevölkerung (ab 65 Jahre) von Neuruppin und einigen umliegenden Gemeinden. Hierbei soll das sinnliche Wahrnehmungsvermögen (vor allem Hörfähigkeit, aber auch Geschmacks- und Geruchssinn) dieses Bevölkerungsteils mittels unterschiedlicher Tests longitudinal möglichst vollständig erfasst werden. Das so entstehende Register soll regelmäßig erneuert werden und dient als Plattform für Interventionsstudien verschiedener innovativer Designs im Bereich der Sinnesforschung. Derzeit befindet sich das Projekt noch in einer frühen Planungsphase.

 

FLASH- und Minibeam-Radiotherapie (ExVivoStrahl)

Gemeinsam mit dem Deutschen Elektronen-Synchrotron (DESY) in Zeuthen erforschen wir die Kombination zweier neuer Strategien der Strahlentherapie zur Krebsbehandlung. Bei der FLASH-Bestrahlung erfolgt die Strahlung mit stark erhöhter Dosis in stark verkürzter Zeit. Die Minibeam-Bestrahlung erzeugt eine räumliche Modulation der Strahlenintensität. Da es sich um experimentelle und neuartige Therapien handelt, erfolgt die Evaluation zunächst ex vivo an präpariertem Lungengewebe von Labormäusen, später dann in vivo in Labortieren und schließlich ex vivo an menschlichem Gewebe. Das Projekt wird finanziert durch das Ministerium für Wissenschaft, Forschung und Kultur (MWFK) und die Investitionsbank des Landes Brandenburg.

 

Lebensqualitäts-adjustierte Überlebenszeitanalyse

Onkologische und kardiologische klinische Forschung befassen sich stets mehr mit der Deeskalation etablierter Therapien, vor allem für Patientengruppen mit günstiger Heilungschance. Hierbei wird im besten Fall typischerweise eine leicht reduzierte Heilungschance einer deeskalierten Therapie subjektiv gegen ein verbessertes Nebenwirkungsprofil abgewogen, um zu einer Entscheidung für eine optimale zukünftige Therapie zu kommen. Um diese Abwägung zu objektivieren, können Heilung und Lebensqualität in der Analyse formal kombiniert werden durch eine Gewichtung beider mittels sogenannter Utilities, z.B. durch Disability Adjusted Life Years (DALYs). Wir evaluieren existierende statistische Ansätze in Simulationsstudien und anhand von Daten mehrerer abgeschlossener klinischer Studien. Das Projekt befindet sich in der Planung.

 

Ökologischer und Confounding Bias in aggregierten Daten zum Zusammenhang zwischen Verfügbarkeit gesundheitlicher Versorgung und der Häufigkeit chronischer Krankheit

Um den Zusammenhang zwischen der Verfügbarkeit gesundheitlicher Versorgung und der Häufigkeit chronischer Krankheiten zu untersuchen und dabei den Einfluss von Lebensgewohnheiten zu kontrollieren, eignen sich prinzipiell öffentlich verfügbare regional aggregierte Daten zu Krankheitsinzidenz, gesundheitlicher Versorgung und Lebensgewohnheiten. Allerdings ist unklar, wie die Analyse solcher Daten korrigiert werden kann um den Einfluss von Lebensgewohnheiten sowie den ökologischen Bias, der durch die räumliche Aggregation entsteht. Mit einer Simulationsstudie sowie mit Daten des Bundeslands Brandenburg analysieren wir Richtung und Ausmass der Verzerrung und mögliche Ansätze zur Korrektur. Das Projekt befindet sich in der Planung.

 

Statistische Methoden für die Bewertung später Nebenwirkungen von heutiger Radiotherapie

Ein Verständnis der Risiken von Langzeitnebenwirkungen nach Radiotherapie zur Krebsbehandlung ist relevant für die Therapiewahl, die Information der Patienten und die mögliche Vorsorge durch Screening. Empirische Schätzungen beruhen wegen des langen Follow-ups zur Beobachtung der Spätfolgen in der Regel auf historischen Therapiemethoden. Während vor 20-30 Jahren große Gebiete mit hoher Dosis bestrahlt wurden, ist die Radiotherapie heute sehr fokussiert auf die befallenen Gebiete und benutzt deutlich niedrigere Strahlendosen. Für diese modernen Therapiemethoden gibt es noch keine Studien mit ausreichend langem Follow-up. Wir entwickeln die Methode des Risikotransfers, um durch Segmentierung der Dosisverteilung im gesunden Gewebe dennoch empirische Daten zur Abschätzung von Risiken für heutige Radiotherapieexpositionen zu benutzen. Am Beispiel von Brustkrebs nach Hodgkin-Lymphom haben wir basierend auf der dreidimensionalen Verteilung der Strahlendosis in der Brust von Patientinnen, die vor langer Zeit behandelt wurden und über eine langes Follow-up verfügen, entsprechende statistische Modelle für moderne Radiotherapiemethoden entwickelt. In Simulationsstudien untersuchen wir die Eigenschaften dieser Modelle. Unser Ziel ist, für verschieden Krebsarten neuartige statistische Modelle aus empirischen Daten zu entwickeln, die sich eignen zur Schätzung der Risiken nach den sich stets weiterentwickelnden Radiotherapiemethoden.
